What Is a Federally Qualified Health Center (FQHC)?

Federally Qualified Health Centers are community-based healthcare organizations that provide
comprehensive primary care services.

FQHCs are committed to:

Do I Need Health Insurance?

No.

As a Federally Qualified Health Center (FQHC), we provide care regardless of a person's insurance status or ability to pay.

We accept many insurance plans, including:

  • Medicaid
  • Medicare
  • Commercial insurance plans
  • And more!

For individuals without insurance, financial assistance and sliding fee discount programs may be available based on eligibility.

Can I use the Mobile Medical Unit if I am covered by my parents’ health insurance?

Yes! 

Students who are covered under a parent or guardian’s health insurance can still access healthcare services on the Mobile Medical Unit. Your insurance coverage does not prevent you from receiving care. 

When you visit, you will provide your insurance information, and we will bill your insurance for covered services. If you are concerned about privacy, confidentiality, or whether your parent/guardian will receive information about your visit, please speak with a member of our staff. We are happy to explain
your options and help you understand how your insurance may be used.

What is the Sliding Fee Discount Program?

As a Federally Qualified Health Center (FQHC), we are committed to ensuring that healthcare is accessible and affordable for everyone. Our Sliding Fee Discount Program helps reduce the cost of services for eligible patients based on household size and income. 

This program is available regardless of insurance status and is designed to help patients receive the care they need without financial barriers.

Who is eligible for the Sliding Fee Discount Program?

Eligibility is generally based on:

  • Household income
  • Household size
  • Current Federal Poverty Guidelines

Patients may qualify for discounted healthcare services if their household income falls within the program's eligibility guidelines.

Do I have to be uninsured to qualify?

No.

Both insured and uninsured patients may qualify for the Sliding Fee Discount Program if they meet income eligibility requirements.

The discount applies to eligible services provided by our health center and on our Mobile Medical Unit. 

How do I apply?

Applying is simple. You will be asked to complete a Sliding Fee Discount application and provide documentation that verifies your household income and household size.

Our staff can assist you throughout the application process.

What documents may be needed?

Examples of acceptable documentation may include:

  • Recent pay stubs
  • Tax returns
  • Social Security benefit statements
  • Unemployment benefit statements
  • Pension or retirement income statements
  • Other proof of household income

If you are unable to provide standard documentation, please speak with our staff about alternative options.

How much will I pay?

The amount you pay depends on your household income and household size.

Patients who qualify may receive services at a reduced cost (or free) according to our Sliding Fee Discount schedule.

A staff member can explain your specific discount level and any applicable fees before services are provided.

If I have insurance, should I still apply?

Yes.

Even if you have health insurance, you may qualify for assistance with certain out-of-pocket costs depending on your eligibility and our program guidelines.

We encourage anyone concerned about healthcare costs to ask about the Sliding Fee Discount Program.

How often do I need to renew my application?

Sliding Fee Discount eligibility must be renewed periodically (at least annually) to ensure that the information on file remains current.

Our staff will let you know when it is time to update your application and documentation. 

Will applying affect my healthcare services?

No.

Applying for or receiving a Sliding Fee Discount will not affect the quality of care you receive. All patients receive the same high-quality healthcare services regardless of insurance status, income level, or ability to pay.

Is my financial information kept confidential? 

Yes.

All information provided during the application process is kept confidential and is used only to determine eligibility for the Sliding Fee Discount Program and related services. 

What if I am not sure whether I qualify? 

We encourage you to apply.

Many people are surprised to learn that they may be eligible for discounted services. Our staff will review your information and explain all available options.
